   The Hauppauge Fire Department was dispatched to a motorcycle vs. truck motor vehicle accident. Engines 14, 3 & Heavy Rescue 3-8-9 operated at this scene under the command of Assistant Chief Thomas Kriklava. F.D. personnel assisted Islip Town Paramedic and the Suffolk Police Paramedic and evaluated the tank truck (carrying Liquefied Nitrogen) for any damage. There was none. The driver of the truck was uninjured. The driver of the motorcycle was believed to have sustained multiple injuries and transported by helicopter by the Suffolk County Police Aviation Bureau to Stony Brook Hospital. Many thanks to Brentwood Legion Ambulance who responded with 3-25-16 to fill in for the responding Central Islip Ambulance who's response was cut short by a traffic accident while enroute. There were only minor injuries reportedly at this accident. All F.D. Units were secure by 1337 Hours.

   The Hauppauge Fire Department was alerted for a vehicle fire at the end of Cabot Ct. Arriving units found a 2002 GMC dump truck used in the landscaping business in flames. An attached equipment trailer was undamaged. Engines 3-8-3, 3-8-4 and Rescue 3-8-9 operated at this scene under the command of Chief Gagliardi. This fire and circumstances surrounding it were made known to the Suffolk County Police Arson Squad for investigation. All Fire Department equipment were secure by 0445 Hrs.

   The Hauppauge Fire Department was alerted for a Motor Vehicle Accident at Motor Pkwy. and Blydenburgh Rd. involving a 1990 Mercury and a 1991 Ford. Fire Department units operating at the scene were Heavy Rescue 3-8-9, Engines 3-8-4 & 3-8-14 under the Command of Chief of Department Michael Gagliardi. It was necessary to disentangle the male driver of the 1990 Mercury so he could be treated by the Islip Town Paramedic and by the staffs of Central Islip-Hauppauge Ambulance and the Lakeland Fire Department Ambulance. Firefighters removed the drivers door, the vehicle roof and cut the steering wheel. The female driver of the 1991 Ford was transported to Saint Catherine's Hospital by CI-Hauppauge Ambulance 3-26-16. Lakeland Fire Department Ambulance 3-13-9 transported the victim extricated from the 1990 Mercury to Southside Hospital. All Fire Department equipment was back in service as of 1803 Hrs.

Mutual Aid - Structure Fire, Croft Lane, Smithtown - 07-07-04 at 1343 Hrs.

   The Smithtown Fire Department responded to a working structural fire on Croft lane in Smithtown. Smithtown was assisted by the Kings Park, Saint James, Commack, and Hauppauge Fire Departments at the scene. Under the Direction of Chief of the Smithtown Fire Department Timothy Crafts and 1st Assistant Chief Jack Hartford the personnel dealt with not only a rather serious house fire but two 100 Pound Liquefied Petroleum Gas Tanks with relief valves operating due to direct flame impingement. There was no one home at the time of the fire and there were no significant injuries to any firefighters. The origin and cause of the fire is under investigation by the Suffolk County Police Arson Squad and the Town of Smithtown Fire Marshals Office.

   The Hauppauge Fire Department was alerted for a motor vehicle accident (rollover) at the corner of Old Willets Path and Rutland Gate. Engine 3-8-14 was on scene and found a 1997 Toyota Camry on it's drivers side. The lone female occupant had already self extricated and appeared to be only shaken ant not injured. The crew of Central Islip-Hauppauge Ambulance 3-26-15 transported the vehicles driver to Saint Catherine's Hospital where she could be further evaluated. Engine 3-8-14 remained on scene until Suffolk County Police Emergency Service unit #4 arrived. With our assistance, SCPES uprighted the vehicle and placed back onto the roadway. Unit 3-8-14 was back in service as of 1329 Hrs.

   The Hauppauge Fire Department was alerted for a motor vehicle accident on the Northern State Parkway, eastbound just west of the Veterans Highway junction. Engines 3-8-3, 3-8-14, Heavy Rescue 3-8-9 operated at this scene under the command of Assistant Chief Thomas Kriklava. A 2002 Lexus Rx 300 had left the roadway and careened into the woods adjacent to the road and overturned onto its driver side. The crew of Engine 3 utilized rope to stabilize the vehicle while the ambulance crew from Commack Ambulance 2-16-17 treated the injured female driver. No extrication work was required as ambulance personnel elected to remove the victim through the vehicles sun roof. The patient was transported to Good Samaritan Hospital by Commack ambulance. All Fire Department units were in service as of 1919 Hours

   The Hauppauge Fire Department was alerted for a car fire on the Eastbound Northern St. Pkwy, 1/4 mile west of Veterans Memorial Hwy. Engine's 3-8-3, 3-8-4, 3-8-14 and Heavy Rescue 3-8-9 responded to the scene under the command of Chief Thomas Kriklava. Arriving units found a 1990 Toyota Camry with an advanced engine compartment fire. A single 1 inch and 3/4 hose line from Engine 3-8-3 was used to extinguish the fire. Cause of the fire was to believed to be an unspecified mechanical failure. All units were back in service at 2237 Hrs.

   The Hauppauge Fire Department was alerted for a car fire in the vicinity of Northern St. Pkwy. and Veteran's Hwy. On the arrival of Chiefs Gagliardi, Woods and Kriklava they found a Nissan Infiniti had left the road at a high rate of speed and careened (overturning several times while taking out some large trees in the process) through the woods almost reaching Veteran's Hwy. Suffolk County Police were already on the scene and had used there portable extinguishers to put out the fire in the engine compartment. The lone male occupant had apparently self extricated and was uninjured. Engine 3-8-4 Operated at this scene utilizing there booster hose to ensure extinguishment of the engine compartment. All units were secure from scene at 0346 hrs.

   The Hauppauge Fire Department was alerted for structural fire at #73 Grissom Way. Engines 3-8-3, 3-8-4, 3-8-14, Ladder 3-8-7 and Heavy Rescue 3-8-9 operated at this scene under the command of Chief of Department Michael Gagliardi. All occupants had self-evacuated. Companies encountered a moderate to heavy fire condition in the Garage of this two story residence with extensive smoke throughout. Three lines were stretched to contain the fire which was declared under control at 0535 Hrs. The cause of this fire is under investigation by the Suffolk County Police Arson Squad. Many Thanks to Commack Fire Department for sending a rapid intervention team and for Smithtown Fire Department for filling in at Hauppauge Headquarters. All Fire Department equipment was back in service by 0714 hrs.

   The Hauppauge Fire Department was alerted for a Motor Vehicle Accident with reported entrapment at the intersection of Veterans Highway and Old Willets Path. On arrival Chief Officers were confronted with a four vehicle accident with the worst of the vehicles being a 2000 Kia with the female driver seriously injured and trapped in the vehicle. Equipment that operated at this scene were Heavy Rescue 3-8-9, Engines 3-8-3 & 3-8-4. Engine 3-8-14 stood-by at a nearby helicopter landing zone as the patient was to be airlifted to Stony Brook Hospital Trauma Center. Ambulances on scene were Central Islip-Hauppauge 3-26-17 and Smithtown F.D. 4-2-61. A number of Hydraulic spreaders (hurst tools) were put to work at this incident to provide access for medical personnel and to disentangle the victim. Suffolk County Police Emergency Services Units also assisted in this task. The overall operation was conducted under the Joint Command of Fire Chief Michael Gagliardi and the Ranking Suffolk County Police Officer on Scene. All Fire Department Units were in service as of 0645 hrs.

   The Hauppauge Fire Department was alerted for an Motor Vehicle acccident at Veterans Highway and Gatewood Drive. Engine 3-8-3 was on scene under the command of Assistant Chief Kriklava. Commack Ambulance personnel were already on scene. The female driver was to be transported to Saint Catherines Hospital. This vehicle apparently left the roadway. All Hauppauge units were in service as of 1834 Hours.

   The Smithtown Fire Department Requested the Hauppauge Fire Department respond to the scene of a working structural fire at 23 Mill Dam Road. The request for aid was for a Rapid Intervention Team. Heavy Rescue 3-8-9 responded under the command of Assistant Chiefs Woods & Kriklava. Other Departments operating at the scene were Kings Park and Saint James. Ambulances also came from Nissequogue & Commack Volunteer Ambulance. There were some minor injuries to firefighters. The cause of the fire was being investigated by the Suffolk County Police Arson Squad and the Smithtown Fire Marshals Office. As you can see the house sustained substantial damage.    The Hauppauge Fire Department was alerted for a Motor Vehicle Accident in front of 250 Rabro Drive. A Chevrolet Astro Van struck the rear of a semi-trailer. The lone male driver suffered a leg and shoulder injury. CI-Hauppauge Ambulance 3-26-18 was on the scene. All F.D. units were back in service by 1302 hrs. Assistant Chief Woods was the Officer in Charge.

   The Hauppauge Fire Department was alerted for an MVA with extrication required at Motor Parkway & Hoffman Lane. Under the Command of the three chiefs the crew of 3-8-9 operated on a Toyota Echo to facilitate the removal of an injured female driver. The Islip Town Paramedic and CI-Hauppauge ambulance 3-26-18 were on scene. All F.D. units were secure by 0802 hrs.

   The Hauppauge Fire Department was dispatched for a car fire in the Parking lot of the Hidden Pond Park/The Rinx. Engine's 3-8-4 & 3-8-14 operated at this scene under the command of 2nd Assistant Chief Thomas Kriklava. The fire involved a 1989 Chevrolet Celebrity. The fire appeared to be of accidental nature, but there was no one at the establishment to claim the vehicle. Therefore the incident was turned over to the Suffolk County Police and the Islip Town Park Ranger for further investigation with the owner after they have located him/her. All equipment was secure by 1828 hrs.

   The Hauppauge Fire Department was was alerted for another MVA with entrapment at the intersection of Motor Parkway & Kennedy Drive. Under the Command of Chief of Department Gagliardi Engines 3-8-4, 3-8-14 & Heavy Rescue 3-8-9 operated at this incident. One male passenger of a 2002 ford which had been tangled with a tractor trailer was injured and required extrication. CI-Hauppauge Ambulance 3-26-17 treated and transported the injured party. All equipment was in service by 1417 hours.

   The Hauppauge Fire Department was alerted for a motor vehicle accident on the westbound LIE at Exit 55. Under the Command of Chief of the Department Gagliardi Engines 3-8-4, 3-8-14 and Heavy Rescue 3-8-9 operated at this scene. Suffolk County Police Emergency Services unit also operated at this scene. Due to the usual location confusion on the Long Island Expressway we also had assistance from the East Brentwood Fire Department who supplied an ambulance for one of the injured parties and one ambulance (3-26-17) from Central-Islip Hauppauge Ambulance. The 1984 Jaguar struck in the rear by a 2002 Ford Pick-up sustained very serious rear end damage. Extent of Injuries to vehicle occupants was unknown by this author. Traffic on westbound LIE had to be diverted off for approximately 20 minutes which made for a traffic nightmare for drivers stuck in it. All Fire Department units were secure by 1341 hrs.


Kitchen Fire - Fairfield/New England Village Apartments - 03-24-04 at 1829 Hrs.

   The Hauppauge Fire Department was alerted for a structural fire at the Fairfield/New England Village apartments on Lincoln Boulevard. The crews of Engines 3-8-3, 3-8-4 and 3-8-14 under the command of First Assistant Chief Michael Gagliardi operated at this kitchen fire that had already been extinguished by civilians. Firefighters removed the heavily damaged microwave which had been located above stove top (point of origin) to the exterior of the building and ventilated the apartment with the use of natural and mechanical ventilation fans. The cause of this fire was cooking oil left on stove top unattended. All units were back in service by 1845 hours.

   The Hauppauge Fire Department was alerted for a Motor Vehicle Accident at Veterans Highway & Ledgewood Drive. Engine 3-8-3, Rescue 3-8-9 operated at the scene under the Command of Chief of Department Louis Zara. This Accident involved three vehicles with two persons being injured. The two injured were transported to the hospitals by two Commack ambulances that were promptly on the scene. Suffolk County Police Emergency Services unit #3 in conjunction with Firefighters from Rescue 3-8-9 operated on the 2004 Volvo (door pop) to facilitate medical treatment and patient removal. All vehicles involved, a 2004 Volvo, A 2004 BMW, and a 2004 Subaru were all heavily damaged. All Fire Department units were secure as of 2301 hrs.

   The Hauppauge Fire Department was alerted for a car fire on the Westbound LIE at the Exit 55 off ramp. On arrival they found a heavily involved 1994 Toyota Camry which apparently had suffered some type of catastrophic mechanical failure. Two vehicle occupants were uninjured. Engine 3-8-14 and Rescue 3-8-9 operated at this scene under the command of Captain John Kogel.

   The Hauppauge Fire Department was alerted for a motor vehicle accident with possible entrapment and/or ejections in vicinity of 1600 Motor Parkway. Rescue 3-8-9, Engine 3-8-11, Engine 3-8-14 operated at this scene under the command of Captain John Kogel. Three vehicles were involved in this collision with nine persons being injured. There was no entrapment but a number of people were ejected and seriously injured. One person was taken by helicopter to Stony Brook Trauma center by the Suffolk Police. Many thanks to Central Islip - Hauppauge Ambulance, Lakeland Fire Department Ambulance, Nesconset Fire Department Ambulance, Smithtown Fire Department Ambulance and the Exchange Ambulance of the Islip's for their prompt response. Also many thanks to the Three Islip Town Paramedics who responded promptly. All Hauppauge units were secure by 1107 hrs.


MVA - Motor Pkwy. & Express Dr. S. - 02-09-04 at 1813 Hrs.

   The Hauppauge Fire Department was alerted for a Motor Vehicle Accident with entrapment at the intersection of Motor Parkway and Express Drive South. Rescue 3-8-9, Engines 4 & 14 operated at this scene under the command of Assistant Chief Michael Gagliardi. The crew of rescue 9 with assistance from the crew of Engine 4, manned by company one firefighters performed a door displacement on a 1992 Chrysler Eagle so ambulance personnel could treat and remove the injured female passenger. The driver of this vehicle was also injured. Both injured parties were treated and transported to the hospital by the crew of Central-Islip Hauppauge Ambulance 3-26-16. The other vehicle involved a 1997 Ford Expedition sustained minor to moderate damage and the occupants reportedly refused medical attention. All Fire Department units were secure from this incident by 1833 Hrs.

   The Hauppauge Fire Department was alerted for a motor vehicle fire in the vicinity of 350A Motor Parkway. The responding units, Assistant Chief Ray Woods & Engine 3-8-14 found a 1985 Pontiac 6000 with the engine compartment heavily involved in fire in the driveway of the fast-food establishment at this address. They quickly quelled the fire using a single hoseline. The owner/operator of the vehicle stated she thought there was something wrong while waiting at the drive-in window and while starting to leave had smoke and fire emit from under the hood. She very quickly evacuated the car.The fire which was confined to the engine compartment area and front wheel area appeared to be of accidental origin. All fire department units were secure by 1410 Hours.

   The Hauppauge Fire Department was alerted for an motor vehicle accident with possible fire. Chief Officers Gagliardi & Woods and Engine 14 & Rescue 9 immediately responded to the scene. They found a 3 car accident with no fire and some minor injuries. At least two of the vehicles was heavily damaged. Ambulance personnel from Central Islip-Hauppauge ambulance attended to the aided. F.D. personnel applied absorbent, removed debris from roadway, disabled vehicle electrical systems as needed and illuminated the scene. All units were secure by 1937 hrs.

   The Hauppauge Fire Department was dispatched for a motor vehicle accident with entrapment at Motor Parkway and Express Drive North (There seems to be one here almost every week). Rescue 3-8-9, Engines 4 & 14 operated at this scene under the command of Assistant Chief Michael Gagliardi. F.D. assisted CI-Hauppauge Ambulance personnel in extricating one victim from 2000 Lincoln that had wedged itself up against the overpass guardrail. There were two or three other aided that were attended to by combination of Ambulance and Fire Department personnel. The other vehicle involved was a 1996 Ford private ambulance. The exact circumstances of this accident were unknown by this author. All units were secure from this scene by 1112 hrs.

   The Hauppauge Fire Department was alerted for a Motor vehicle Accident with entrapment and multiple injuries. Engines 3-8-11, 3-8-3, and Rescue 3-8-9 operated at this scene under the command of Chief of the Department Louis Zara. A Ford Taurus one of two vehicles involved left the roadway and entered a wooded area adjacent to the road. Crews utilized spreaders and cutters to displace passenger side doors and cutters to remove vehicles roof. The male occupant in the vehicle was unconscious and apparently seriously injured. Many thanks to East Brentwood Fire Department and Lakeland Fire Department for assisting Central Islip Hauppauge Ambulance by providing additional ambulances. All Fire Department units were secure from scene by 1752 hrs.

   The Hauppauge Fire Department was alerted for a vehicle fire in the parking lot of T.G.I Fridays at 3045 Express Drive North. Responding equipment and manpower found a 1996 Chevrolet van style vehicle with heavy fire in the Engine/Passenger Compartment. Engines 14 & 11 operated at this incident under the command of Assistant Chief Ray Woods. This incident was turned over to the Suffolk County Police Arson Squad for further investigation. All units were back in service as of 0240 hrs.

   The Hauppauge Fire Department was alerted for a Motor Vehicle Accident with entrapment in front of # 110 Motor Parkway. Units found a two vehicle accident with a female occupant of a 2000 Kia trapped and possibly seriously injured on the passenger side of this vehicle. F.D. personnel from Truck Company # 1 operating from Rescue 9 utilized Hydraulic Jaws, Cutters and sawzall to remove passenger side of vehicle and roof and operated hydraulic ram to perform a dash displacement. Scene illumination was also provided by Rescue 9's Light tower. F.D. personnel also applied absorbent to spilled fluids and disabled both vehicles electrical systems. The other vehicle involved was a 1992 Toyota. There were a total of 3 persons injured all attended to by members of the Central-Islip Hauppauge Community Ambulance who had three ambulances promptly on the scene. Hauppauge Engines 3-8-3, 3-8-14 and Rescue 3-8-9 operated at the scene under the command of Chief of Department Louis Zara. All units were secure by 2329 Hrs.


Car Fire - 55 Kennedy Drive - 01-15-04 at 1038 Hrs.

   The Hauppauge Fire Department was dispatched for a report of a motor vehicle fire in the parking lot of 55 Kennedy Drive. A Ford Bronco was being utilized for snow plowing when it evidently experienced a serious mechanical problem which resulted in a fire and the destruction of the vehicle. The driver was uninjured but unfortunately did not speak English. The vehicle belonged to a landscaping contracter who had the contract for clearing the parking lot. Engines 3-8-11, 3-8-14, & Rescue 9 operated at this incident under the command of Captain John Kogel. All units were back in service by 1106 Hrs. It was ten degrees fahrenheit at the time of this incident.


MVA - I/F/O 75 Hoffman Lane - 01-06-04 at 2221 Hrs.

   The Hauppauge Fire Department was alerted for a motor vehicle accident at 75 Hoffman Lane. This was a single vehicle accident involving a 1988 Honda Prelude which struck the building at this address. The lone male driver did suffer injuries and was treated and transported by personnel of the Central Islip-Hauppauge Community Ambulance Squad. The Building owner was notified and a referral was to be made to the Village of Islanda Building Inspector but damage to building appeared to be minor. Engine 11, Heavy Rescue 9 operated at this scene under the command of Assistant Chief Ray Woods. All units were secure by 2240 hrs. Shortly after this accident at 2302 hrs Engine 3 and Heavy Rescue 9 under the Command of Captain John Kogel operated at another single vehicle accident involving a car leaving the roadway and striking a tree in front of 157 Adams Avenue injuring the occupant. (Sorry, no photos of this one) Many Thanks to the East Brentwood Fire Chiefs and their crew of their Ambulance 3-3-7 for handling this call in the absence of Central Islip-Hauppauge Community Ambulance who were unable to respond due to their work load at that time. All Hauppauge units were secure from this second incident by 2223 hrs.

   The Hauppauge Fire Department was alerted for an MVA with reported entrapment in front of 220 Veterans Hwy near the State Office Building Entrance. Fortunately for all occupants in the 1989 Oldsmobile and the 2003 Mitsubishi in this rather severe accident resulted in no entrapment or injuries. All persons involved refused medical attention to police, ambulance and fire personnel. Fire Department personnel removed debris from roadway and applied absorbent to spilled fluids. The units on scene Rescue 3-8-9, Engines 11 & 3 were under the command of Chief of the Department Louis Zara. All Fire Department Equipment was back in service by 2002 hrs.
